Piles, also known as hemorrhoids, are enlarged or symptomatic vascular cushions in and around the anal canal. They may cause bright-red bleeding, itching, discomfort, mucus discharge or a lump that comes out during bowel movements.
Many patients with early hemorrhoids improve with dietary changes, increased fibre, adequate fluids and treatment for constipation. Office-based procedures or surgery may be considered when symptoms persist, prolapse becomes significant or conservative treatment does not provide adequate relief. Current guidelines recommend dietary and behavioural measures as first-line treatment for symptomatic hemorrhoids.
Dr. Sushant Wadhera provides evaluation and treatment planning for piles and other ano-rectal conditions in Delhi according to the grade of hemorrhoids, symptoms and individual clinical findings.
Painless Surgery for Piles/ Hemorrhoids is an excellent treatment option for this very common disease.

Hemorrhoids
Piles/ Hemorrhoids are cushions of blood in the lower rectum.
They are given by nature to protect the rectal lining from hard stools and to protect the anal sphincter.
These cushions aid in closure of the anal canal during increased abdominal pressure.
Also help in sensory feedback between liquids and gas.
Hemorrhoid disease exists only in HUMANS, as we have a rectum and an anal canal pointing vertically down so the venous pressure is high.
During straining to pass motion as in chronic constipation, hemorrhoidal cushions get congested.
Hard stools cut these congested cushions and fresh bleeding ensues.

Grade 1 : Only congested piles, not coming out of anus.
Grade 2 : Droop down and reach the anus. Initially come out on straining, but go back in themselves.
Grade 3: They come out without straining and need to be pushed back.
Grade 4 : Prolapsed / thrombosed piles.
A patient with hemorrhoids maybe also be having a FISSURE IN ANO .

Stapler Hemorrhoidopexy
Stapled Hemorrhoidopexy: This technique is very efficient more most of the hemorrhoid cases. It takes care of mild prolapse of the rectal mucosa which occurs in chronic cases. It is also called as ANAL FACE LIFT.
GOLD STANDARD of piles surgery reserved for severe and recurrent cases.
Dr. Sushant Wadhera provides evaluation and surgical treatment planning for patients with piles and other ano-rectal conditions in Delhi.
Treatment is selected according to the grade of hemorrhoids, severity of bleeding or prolapse, presence of an external component, previous treatment and overall health, rather than using the same procedure for every patient.

1. What are piles or hemorrhoids?
Piles, also called hemorrhoids, are swollen or enlarged vascular cushions in and around the anal canal. They may cause bleeding, itching, discomfort, mucus discharge or prolapse during bowel movements.
2. What are the common symptoms of piles?
Common symptoms include bright-red bleeding during or after passing stool, anal itching, swelling, discomfort, mucus discharge and a lump that may come out during bowel movements.
3. What causes piles?
Piles may be associated with chronic constipation, excessive straining, prolonged sitting on the toilet, pregnancy, obesity, heavy lifting and repeated increased pressure inside the abdomen.
4. Do all piles require surgery?
No. Many early hemorrhoids can improve with increased fibre, adequate fluids, treatment of constipation and better bowel habits. Surgery is usually considered for persistent, recurrent or advanced hemorrhoids.
5. What are the grades of internal hemorrhoids?
Grade 1 hemorrhoids remain inside the anal canal. Grade 2 prolapse during bowel movements but return on their own. Grade 3 usually need to be pushed back manually, while Grade 4 remain prolapsed and cannot be manually reduced.
6. How are piles diagnosed?
Diagnosis usually involves medical history and examination of the anal and rectal area. Digital rectal examination, anoscopy or proctoscopy may be performed depending on symptoms.
7. Is rectal bleeding always caused by piles?
No. Rectal bleeding can occur due to several other anorectal and gastrointestinal conditions. Persistent, unexplained or unusual bleeding should be properly evaluated instead of being assumed to be piles.
8. What is rubber band ligation for piles?
Rubber band ligation is an office-based procedure in which a small rubber band is placed around the base of an internal hemorrhoid. This cuts off its blood supply, causing it to shrink and eventually separate.
9. What is hemorrhoidectomy?
Hemorrhoidectomy is a surgical procedure in which symptomatic hemorrhoidal tissue is removed. It may be considered for large or advanced hemorrhoids, especially when other treatments are unsuitable or have not worked.
10. What is stapled hemorrhoidopexy?
Stapled hemorrhoidopexy is a surgical procedure that repositions prolapsing internal hemorrhoidal tissue inside the anal canal. It is suitable only for selected patients and is not the preferred procedure for every type of hemorrhoid.
11. Is laser treatment for piles completely painless?
No. Laser-based procedures may be associated with less discomfort in some patients, but no piles procedure can be guaranteed to be completely pain-free. The appropriate treatment depends on the grade and type of hemorrhoids.
12. How long does recovery take after piles surgery?
Recovery depends on the type of procedure. Office-based treatments usually have a shorter recovery period, while recovery after surgical hemorrhoidectomy may take several weeks.
13. Can piles come back after treatment?
Yes. Recurrence is possible, particularly if constipation, straining or poor bowel habits continue. Maintaining adequate fibre, hydration and regular bowel habits may help reduce recurrence risk.
14. When should piles be treated urgently?
Seek prompt medical care if you have heavy or continuous bleeding, severe anal pain, fever, pus discharge, large blood clots, dizziness or weakness associated with bleeding.
15. Who should I consult for piles treatment in Delhi?
Patients with rectal bleeding, prolapse, persistent discomfort or suspected hemorrhoids can consult a surgeon experienced in ano-rectal conditions. Dr. Sushant Wadhera provides evaluation and treatment planning for piles in Delhi.
