DIFFICULT GALL BLADDER SURGERY/ XANTHOGRANULOMATOUS CHOLECYSTITIS
Xanthogranulomatous cholecystitis is an uncommon variant of chronic cholecystitis characterized by xanthogranulomatous inflammation of the gallbladder with thickened gallbladder wall with multiple yellow-brown nodules form in the gallbladder wall. The inflammatory process can extend into the gallbladder wall and adjacent structures like liver bed. There is accumulation of fibrous tissue in the gallbladder wall makes holding and surgical dissection around gall bladder very difficult and so there is high risk of surgical complications.
So, Xanthogranulomatous cholecystitis resembles like gall bladder malignancy even though it is benign and is associated with surgical complications. It usually leads to conversion of laparoscopy to open surgery and injury to liver, bile duct and intestines.
